04 JAN 13 - Went to ER for Severe Leg Pain and Numbness/ MRI found L5-S1 Severe Herniated Disc Into Sciatica Nerve.
06 JAN 13- Follow Up with On Post Orthopedic Doctor, Put Bed Rest along with Physical Therapy and Recommendation for Surgery.
25 JAN 13- Surgery. Laminectomy and Discectomy performed.
28 JAN 13- Cleared to Fly After Post Surgery Follow up.
01 FEB 13- Left Fort Lee, VA For 30 Days CON Leave
10 MAR 13- Back To Post From CON Leave. (Milwaukee, WI)
11 MAR 13 -Follow up With Off Post Surgeon in Richmond VA
14 MAR 13- Follow up with ON POST Orthopedic Dr. to find out what happens next (Ft. Lee VA) @0700
14 MAR 13- Had MRI / Post MRI for successful surgery. MEB Board Referral and Packet filled out. Put on PERMANANT PROFILE/ DEAD MANS PROFILE.
15 MAR 13- 0630 Phase 1 Physical for Blood Draw/Vision/Hearing/ Like MEPs.
CURRENTLY AWAITING ON CO TO FILL OUT ASSESMENT PACKET AND THEN BRING TO MY CASE REFERAL FOR MEB ON 18 MAR 13 FOR THE PSYICIAN TO REVIEW for FIT/UNFIT ( WHITCH THE ORTHO PRINTED OUT A AR MANUAL THAT SHOWS MY CONDITION AS UNFIT)
3 MONTHS PHYSICAL Therapy to Try to stay in the ARMY.
12 NOV 13- Reviewed NARSUM, signed it, Being MED boarded for CHRONIC LOWER BACK PAIN. Packet Electronically sent to D.C. for ratings, was told it could take up to 40 Days. Then I see the VA which " Doesn't take long" <------HA What a joke.
