Physical Evaluation Board Appeal

In 1968 I joined the Army. I was disqualified because of previous spinal surgery. I appealed and was commisioned.

I did well in Officer Training and was stationed to Fitzsimons Army Hospital. After several months, I began to develope low back pain and pain down my leg.

I was ordered before a PEB and was found disqualified to serve. I was not granted severence or a disabilty rating.

Recently, I located a document that seemed strang to me. The form is Form 66. Under section 10, assignment limitations, was written NO LIMITATIONS.

At some point without my knowledge or my concent, this limitation was scratched through and replaced with no prolonged standing, no lifting over 15ils, no stooping or crawling, no prolonged walking, no assigment to combat zones.

Had I been aware of these limitation, I would have appealed the PEB results.

Can anyone tell me what to do to appeal the PEB findings?
