Applying for CRSC first time

Good afternoon - I am going to need some guidance figuring out how to submit my CRSC application. I am retired Air Force.

I already went through the VA disability process and was awarded 100% Permanent and Total.

I have downloaded the DD Form 2860 to apply for CRSC. I am confused as to what the other required documents ARE though: VA Physicisans Reports, VA Letter, Ratings Decisions, and VA Code Sheet(s).

I have the letter I received from the VA spelling out, "Service connection for blablabla is granted with an evaluation of 30% effective 1 October XXXX."

Overall I am simply confused about exactly what it is I am supposed to be submitting. I have the pdf medical record I think I received from the AF 4 years ago when I retired but I can't find the password to get in to it. Am I supposed to be using that medical record or should I be getting one from the VA? I will keep working on figuring out how to get into the medical record pdf.

I am unsure as to what else I can/should provide. I would really appreciate some guidance from anybody willing to give it, thanks in advance.

Page 3 (pdf page 4) lists all of the required documents, as for clinical documents, only submit records from the point of injury, i.e., MEDEVAC/CASEVAC, ER report, sick call slip(s), etc. NOT the entire course of treatment. So, yes, "that" record. VA documentation, such as DBQs, doc reports, etc, have no weight on the branch's determination for CRSC. For BH-related conditions, submit the write-ups (if available) for any/all combat-related awards, even if awarded by other branches. If you do not have any, reach out to the AF Historian's office as they can help in regards to the documented attacks(s) (within reason) for the locale you were in while deployed.
 
PACT Act only requires the VA Decision Memo and proof you were in the geographical region as specified. Note: orders alone are NOT proof

They ask for code sheet, but you do not need it. I requested mine still waiting, 6 months and counting, comically I got my code sheet faster from CRSC as it was in my decision paperwork.

Yup, that's why I tell folks not to bother. The Navy is the only one I am not 100% certain they request it on their own. I got my code sheet the same way, with my approval memo lol
