Medically retiring soon 70% DOD, 100 VA%. I understand I qualify for free extended SGLI for 2 years. Is this automatic or do I have to apply for it? If so, do I apply now or post ETS date?

You need to find out if the 120-day VGLI waiver applies after the two years are up. When you exit service you can convert your SGLI to VGLI without proof of good health within a 120-day window. Afterwards proof of good health is required and most disabled vets have their applications denied or only approved for really low amounts.
