Have separation date set, possible to still get medical retirement?

I have a current separation date set for Feb 2022. In the past I went through an IRILO due to conditions from complications on a hernia surgery, result was return to duty with a C code to only PCS/TDY to locations with a fixed military treatment facility. I've since been on fitness test exemptions (since 2016), but somehow considered worldwide deployable, although I'm sure I wouldn't meet the reporting instructions in a lot of the AORs.

Since the IRILO, I've also been diagnosed with other conditions (asthma, gerd, chronic sinusitis, and a hiatal hernia, plus a few others). When I separate I will be just over 15 years in service. Is there any way to pursue getting a medical retirement vs a simple separation in my situation (especially with not much time left)? Not sure if it matters, but I am also in the in Air Force. Appreciate any help or advice anyone can provide.
