Honorable discharge to medical discharge

Hi first time posting here.
I’ve served in the Navy and was honorably discharge. I was a victim of MST and suffered with PTSD through out my tour. At the time I didn’t know I had ptsd and was afraid to speak out. I have been diagnosed with ptsd at VA. I would like to know if my honorable discharge can be changed to medical discharge?
If so how can I get started? What forms I need and where to send them?

I am sorry to hear your story. You need to submit a Department of Defense Form 149 to Navy BCMR. The address is on page 2 of the form. The process is about 2 years long.
