14NOV11 Was first seen by ship for back pain and loss of strength right hand and legs
11JAN12- Placed on 1st LIMDU
06JUL12- Placed on 2nd LIMDU
07JAN13 Neurology at NMCSD initiates PEB with 2 diagnosis
(MIGRAINE WO INTRACTABLE MIG 346.00 AND MYALGIA AND MYOSITIS NOS 729.1)
14JAN13 Had meeting with VA rep.
9 other disabling conditions added for consideration
29JAN12- First VA appt
