Activated Reservist potential MEB

I am an IMA reservist on AD orders. I have been on multiple sets of orders for about 3 years. During this most recent set of orders I had increasing pain in my low back which ended up needing surgery. Since surgery my physical health has rapidly declined and I've been hospitalized twice for intractable pain. I now cant even get through a whole day of work without getting a migraine and sometimes I have to wear a neck brace while doing computer work to make it through. On top of that, my surgery failed and I will have to have another more invasive surgery. This is just the worst of it. My orders are set to end 30 Sep and with all my health issues I know I wont be able to get a civilian job. I am leaning heavily towards requesting an IRILO with the hope of getting TDRL. I am fairly confident I can get the minimum 30% with the DOD and I have more than the required minimum years for reserve medical retirement. I have a ton of questions but for now if anyone can give any advice I would appreciate it! My main questions are:
With upcoming surgery that will most likely happen around the end of my orders and require at least 2 months recovery, can I get my orders extended to be able to pay my bills and have my same health care?
and/or If the IRILO is submitted before the end of my orders will I get my orders extended during the process?
I am the sole provider for my family and I cannot be without a job.
 
I had a surgery on my knee, which extended my MEB process. I got notice of the MEB on the same day I had surgery. That put my MEB back by a few months. My orders ended on Dec 31, 2025. Since i was in the MEB Process, they extended my orders to the end of my enlistment with the agreement of when the MEB comes back, they will curtail my orders to end when my MEB says to get out. That's my situation. Yours sounds very similar but you would want to be in the MEB process to extend your orders imo. I am not a medical profession, HR person, or anything like that. I'm just going off of what I went through though so I'd trust but verify my statement for your situation... That is if you are still in also as this OP is almost a year old.
