Already receiving VA disability and now going through MED board

Iv'e been receiving VA disability since 2012, for injuries I incurred on active duty while in the Army. I am currently in the National Guard and am going through a MED board, for one of those injuries. My right knee ACL is re torn and the VA doctor is not recommending surgery because I would need 2 surgeries and still have knee pain, because I now have arthritis in my knee. Ive been given this VA FM 21-0819 to complete and am concerned about 2 things. I was told by my PEBLO that I needed to put all conditions I had profiles for on page 1. She said unless I put it there, the DOD would not consider it for part of my rating from them. While I was on active duty I had numerous back problems and was treated for it and continue to receive treatment. Also on active duty I was told, by a doctor, that I had back problems because my torn ACL went untreated for my entire deployment. So I definitely want my back to be considered by the DOD. But today I received a call from another lady saying my PEBLO was new and wrong. She said I can't add anything to the "referred conditions". She wants me to put it on page 2, which is apparantly the VA side of the VA FM 21-0819. The problem I see with that is that I already have a VA rating for my back. I also have some disabilitys in an "appeal" status with the VA. Does anyone know how to properly fill out this form for my situation? If I don't list current rated VA disabilities on page 2 am I saying that they aren't service connected? If I do list them on page 2 will they all be reevaluated?
 
Thats good. LOD's are important. Claim everything you can for the VA, If you have a lot of pain with the injuries I would claim chronic pain also. Best of luck.
