Doctor Letter

Hello all, currently building my case for my formal board this spring. I am seeing a specialist for the condition that I want to add as a rated condition on the dod side (received 50% VA rating for it) and wanted to see if anyone has any advice for asking this doctor for a letter to better support my case - is there specific verbiage I should ask him to use? Should I give him an example letter to work with?

The key to a DoD rating is that it doesn’t meet the retention standards for your service. On the resource tab of this website you will find your services retention standards.

You don’t mention your branch of service or your condition, so it is difficult to give you more specific recommendations.
