Filling out DA 31? - Transition leave with PTDY

I just got my new ETS date from transpoc and I'm getting ready to fill out my DA Form 31.

Do I fill out two separate leave forms? - one for PTDY and one for terminal leave
or
Do I fill out a single leave form for both PTDY and terminal leave? And if so, how do I fill it out?

My S1 shop is less than helpful.
 
It is to be one DA31, you need to have the following statement in block 17:
I understand that this absence is not directed by and U.S. Government official. I further understand that I cannot conduct public business under this authorization. Accordingly, I will not be entitled to reimbursement for travel, per diem, or any other expenses. I understand that I have the right to cancel it at any time and return to my regular place of duty. PDTY _________ to __________. (You fill in the PTDY dates, they are to be the first days of the requested leave time.)

i.e. your leave time would be 31July to Oct31 and you are taking 20 days of PTDY. So the fill in PTDY dates would be 31July to 19 Aug, and then the regular leave starts on 20 Aug until 31 October.
 
So, what would I put under block 7 "type of leave"?

And for the "accrued" and "requested" leave blocks in block 9, how do I fill those out?
I'll have 40 days of leave accrued and I'm taking 20 days of PTDY - so do I put "40" under "accrued" and "60" under "requested"?

Block 7 would be checked as a normal leave and in the write in section "w/ 20 days PTDY".

The accrued will be the amount you currently have on your LES when you submit the leave form, unless your unit will let you count forward on your new ETS. If they won't allow you to count forward, you will automatically sell the accrued time while on leave. (This is what I had to do.)
You will request 60 and say N/A on Advance Days.
This 60 covers your accrued amount coupled with the PTDY, which is not an advance of days and is not chargable leave.

Then this is what you do:
Take your base pay divide by 30, this equals your daily base pay amount.
Multiply daily base pay amount with # of days accrued (be sure to include amount up to new ETS date), then tax it at 33%. That would be your total.
It is much more benefical to take the leave and adding in the transition leave, that way you get BAS/BAH/COLA allowances with base pay, instead of that measly amount that you'd get from selling it. (That is my opinion... why work for the Army, when you can collect a full paycheck, plus get another job and work there and get the pay???)

Would I now select combination leave and “terminal leave” for type of leave?
