Need Help With Starting A PDRL CASE

Hi all! I'm currently on TDRL and I have a permanent disease. Any help on how to start my PDRL case would be appreciated.

The decision of being moved from TDRL to PDRL lies with the Doctor at the time of your re-evaluation
 
The doctor's "decision" can be appealed!
