SSA-3033 Question..

Recently retired and I submitted my online SSDI application yesterday. I received to fill out ssa-821 and ssa-3033 form. What if my old co-workers do not fill out ssa-3033? Can i continue with my application without ssa-3033? I called the ssa office and told me that they need this form to be completed and submitted with the rest of paperworks. Thanks..

In response to your specific inquiry, if your State SSA DDS don't receive any of the requested information within an established timeframe (e.g., deadline), they shall ultimately continue with the adjudication of your SSA SSDI case file which can significantly effect the outcome unfortunately.

To that extent, no one can "force" your co-workers to complete the requested SSA-3033 form! Instead, please try to emphasize to them the importance of the form for the SSA in reference to your SSDI case file; it can potentially result in a denial of federal disability compensation benefits due to the lack of all available medical evidence.

Thus, I quite often comment that "possessing well-informed knowledge is truly a powerful equalizer."

I was hoping that was not the case if they continue without SSA-3033 form but I did the next best thing I think. Called SSA office and told them that the commander pcs and even the 1SG. I had the acting PSG completed the form. I hope the SSA don't mind because I had the same rank as him. I also added all my profiles from temporary to permanent and the Commanders DA form 7652 which says it all from cannot work from a that date and recommended not to retain me because of profiles..

Good deal on being positively proactive at this point in time! :)

In my opinion, an individual's rank is not as important than the type of specific comments actually annotated in the description of your current medical condition(s) symptomatology.

As such, hopefully, the acting PSG captured the effect(s) of all of your medical conditions which prevent you from performing substantial gainful activity (SGA). In retrospect, to be eligible for federal disability benefits, a person must be unable to engage in SGA.

I hope that is the case for the rank really don't matter. I was talking to a rep from SSA and stated that a co-worker or someone knew about my situation will do even if the commander's not available. The SSA form 3033 is pretty straight forward like yes or no and a few explanation. The only thing I'm a little worried about is putting how many hours I worked a week. I'm just hoping that whomever holding my case, he will will understand that we work 24/7..

No worries & again good deal; it seems that you are on the correct path to properly aide the SSA State DDS in its ability to determine if an individual is capable or not to engage in SGA. ;)
