A bit confused on this example

I was hoping one of you could shed some light on this. This is an old post I had read recently and I don't believe the member is still on the site. But he lists his DOD/VA rating in the first post VA giving him 100. Then lists his MEB Reffered conditions and said his proposed ratings were at 40% VA. I am very confused by this forum post. Is it typical for the VA to shoot a rating up from 40-100 like that? I am at 90% currently and in the med board process and was just wondering if the VA has some sort of different standard beside combined disability (scheduler) that made that individual get to a 100%. They don't request or give TDIU during the Med board process right?

Any guidance is appreciated. Thank you.

if i am reading this correctly: VA rated them 100% for all conditions the 40% was just for the unfitting conditions, as those are just the specific one they posted in that second picture.
 
I see so secondary conditions listed then?
I think they just opted to focus that last post on the unfitting conditions (and how the army dowgraded the VA's Proposal) rather than all conditions. Case and point: I have 11 claimed conditions but the army is only going to rate me on my wrist/forearm.
