Curious as to what will trigger a PEB initiation from the MEB.

What are the specific criteria for your conditions to be submitted to IDES to determine suitability for service?

What branch are you? in the Army once they put you in IDES a MEB will always leads to a PEB.

As far as I know time in service only factors in for dod compensation after found unfit

If you are 20 years or over there are no downsides. You will get your regular retirement + CRDP (if rated 50% or higher by VA) and that is the most compensation you can get dollar for dollar compared to Chapter 61 medical retirement. You could have more net compensation by the fact of not paying federal income taxes by one or more unfitting conditions being designated combat related and the total DOD% (Unfitting conditions) would need to be higher than your earned pension so in your case 60% or higher. If that's not the case there I don't see a benefit to medical retirement but either way you are good to go.
